  • Financial account details
  • Medical information
  • Health insurance information

Who Can File a Claim?

To be eligible for the settlement, individuals must have received a written notice from Brown Integrated Logistics Inc. around September 23, 2024, stating that their personal information was compromised during the data breach from November 13 to November 15, 2023.

Settlement Payment Amounts

Class members may receive compensation in the following categories:

  1. Out-of-pocket costs – Up to $750 for documented expenses such as credit monitoring, identity theft insurance, or other related costs.
  2. Attested time – Up to five hours at $20 per hour (maximum $100) for time spent resolving issues caused by the breach.
  3. Financial losses – Up to $4,000 for proven, unreimbursed monetary losses from identity theft, fraud, or misuse of information linked to the breach.
  4. Credit monitoring services – Two years of free credit monitoring with Experian, Equifax, and TransUnion, including alerts, dark web scanning, identity theft insurance, and assistance services.
  5. Alternate cash payment – Instead of the above benefits, class members may opt for a flat $75 payment.

The maximum total award for eligible individuals can reach $4,850 if combining financial losses, out-of-pocket costs, and attested time.

How to File a Claim

Class members can file a claim online or by mail:

Required Proof and Documentation

To file a successful claim, class members must provide:

  • Class member ID from the settlement notice.
