- Paper form: If completing by hand, follow these steps:
- Print or write clearly.
- Include ZIP codes with addresses.
- Provide full phone numbers (with area code). For international numbers, include the IDD and country code.
- If you don’t remember provider details, check medical bills, prescriptions, or pharmacy records.
- Answer every question. If something doesn’t apply, write “don’t know,” “none,” or “does not apply.”
